At H3C, Care Coordinators are responsible for the care and management of a designated patient population under the direction of our nursing and physician teams.
We’re looking for full or part-time Care Coordinators. You can create your own custom schedule to best suit your needs.
Applicants with a CNA, CMA, or above will receive priority consideration, but all applicants are eligible and will be considered. No healthcare experience is needed. Our expert nursing staff will train you to become a Care Coordinator!
Enrollment Specialists at H3C call patients on behalf of their doctor’s office to explain what their provider’s new Chronic Care Management program has to offer them – better care, more check-ins, and a dedicated team to help them manage their chronic conditions.
Great Enrollment Specialists have call center, reception, or customer service experience.

We hire CNAs, CMAs and above to provide coordinated care services over the phone for our enrolled Medicare patients who are managing multiple chronic conditions. Our nursing team members also provide training to our care coordinators.

Enrollment specialists call patients on behalf of our clinic partners to explain how a chronic care management program can benefit them and to obtain patient consent for program enrollment.
